Dermal Filler FAQs

Dermal Filler FAQs Providing minimally invasive out patient procedures that can produce results similar to surgery makes dermal fillers a go-to. Rejuvenating, refreshing and volumizing, dermal fillers can take years off ones face and give them an added glow. At AIYANA, we love dermal fillers and have had such great success at helping our clients reach their anti-aging goals. When talking about dosing fillers, 1 syringe, 2 syringes, etc, it really is in mLs. And, 5 mls equals 1 tsp. See the diagram below. It is such a small amount of filler, but because of its chemical makeup, the hyaluronic fillers are hydrophilic, meaning they attract water. Some do a better job than others, and because of this they plump up more when inserted in the skin. Types of Dermal Fillers There are a number of them out on the market today, but we will go through the ones we use at AIYANA. Juv�derm is a very popular filler. It is manufactured by Allergan, the same company that manufactures Botox. There are different types of Juv�derm, such as Ultra, Ultra Plus, Voluma, Volbella and Vollure XC. These types of fillers are composed of Hyaluronic gel, a naturally occurring substance in the skin that contributes to volume hydration. It also stimulated the production of connective tissue or collagen that plumps the skin. Restylane is another type of hyaluronic acid filler, similar to Juv�derm, manufactured by Gallderma. Restylane Kysse, Restylane-L, Restylane Lyft and Defyne are all used at AIYANA. Where to Use Filler? There are so many uses for filler and it seems like we are advancing in this treatment so rapidly that filler is being placed all over the face. At AIYANA, the areas around the mouth, the lips, jawline, chin, under eye (tear-trough) and the midface/cheeks and under eyes are the most requested areas. A lot of aging is present in the midface because it is the structure that holds a youthful appearance. When we lose volume in the midface, our lines get deep around our mouth, jowling starts to occur and our eyes look more sunken and dark. Creepiness occurs along the jawline and more fine lines/wrinkles are present in the midface area. By adding filler, this will inhance all of these areas and give an all over refreshed look. Lip Filler When our clients think about lip filler, they are concerned that they will have an outcome that looks overfilled and too plump. At AIYANA, we always try to reassure our clients that it takes a lot of filler, and multiple appointments to become overdone. We are always careful to enhance natural contours, not change appearance. This goes for all filler placement. A refreshed, youthful, glowing end result is always our goal. Before & After Here are some before and afters of our filler clients. What you will notice is that filler placed correctly can make subtle, yet, youthful changes to someones face. It’s refreshing and natural. Lip Filler 1 syringe of Juv�derm Ultra Lower Face/Marionette & Lip Filler 1 syringe of Volbella around mouth, 1 syringe of Juv�derm Ultra in lips AIYANA Rejuvenate 3 syringes of Voluma in cheeks, mid-face, 2 syringes in chin and pre-jowl jawline. {Client had 1 syringe of Juv�derm Ultra in lips 3 weeks prior} Filler FAQs Most filler treatments take 30-45 minutes and require one office visit. Before getting the injections, you may need ice, an anesthetic applied to the skin, or a nerve block (an injection). This varies with the filler and area to be treated. Your provider will inject the filler into the area, often giving you several injections to produce the best results.� You may feel a stinging or burning sensation as the filler is injected.�(from AAD) Post-Care Instructions: Swelling, redness, firmness, itching and tenderness may be present for up to two weeks. Use of cool packs may be used as needed.� Sleep with your head elevated to reduce the amount of swelling. Sleep on your back for the first 48 hours after the procedure.� Do not massage or put pressure on the treated area, unless instructed by injector. Wait 4 hours to apply cosmetics, moisturizer or any lotions on the area. Avoid direct sunlight or extreme cold weather until initial swelling and/or redness has subsided. Do not do heavy aerobic exercise or do anything to raise your core body temperature or blood pressure for 24 hours. The results will be immediate, but its best effects are noted after swelling has resolved. Motrin or Advil, or Tylenol may be used as directed for any discomfort after injection. Cetirizine HCL 10 mg (Zyrtec) and Ranitidine 150 mg (Zantac) may be taken as directed to reduce swelling. Lumps, bumps or irregular, asymmetrical areas may be noted, especially once swelling has subsided. It is normal to feel these lumps and bumps; however, they should not be visible. Any severe swelling, itching, redness, increasing discomfort or discharge from treated area should be reported to your injector ASAP.� Periodic treatments will help maintain the desired level of correction. It can take up to TWO weeks for swelling and any bruising to subside. If additional filler is desired in an area of previous treatment, we require you to wait until the 2 weeks has passed. When Will I See Results? This also varies with the filler. Most fillers fill the skin, so they produce immediate � or close to immediate � results. Our muscles start to thin and spread apart. Our fat pads, that used to almost be 1 fat pad that extended from our temple to our mid face, starts to spread apart. That is why we start noticing hallowing around the eyes, thinning of structure around our mouths, skin that starts to droop and sag around our lower face. All of this occurs, while our skin seems to get bigger. Its like a table with a large table cloth. We add filler to help fill that volume, but it can only take us so far without distorting the face and causing that moon-face shape. This is where Sculptra comes in. The collage treatment we’ve all been waiting for! Sculptra is an injectable sugar molecule that our bodies do not like. When it is injected, our body, over the next month, starts to attack it to get rid of in. During that process, our body produces collagen to help with this removal. As the collagen starts to build, we notice an improvement in our skin texture, tone and laxity. We may also have some volume improvement. This treatment is for everyone, as we are all losing collagen after our 20s. Collagen will last for 3-5 years, but with yearly Sculptra treatments, we can get you aging backwards and turn back the clock. The first Sculptra treatment is like planning the garden. Every treatment after that is where we water and nourish the garden. Kybella KYBELLA��is administered as a series of injections to the treatment area under the chin, destroying fat cells and producing gradual results. Everyone’s chin profile is different, we will come up with a customized approach just for you. The number of treatment sessions will vary based on the amount of fat under the chin and your treatment goals. Microneedling As a huge fan of this treatment, microneedling is a procedure that uses small needles to prick the surface of the skin. This process generates collagen in the skin tissue to help with wrinkles, fine lines, large pores and scarring. The end results include smoother, firmer and more toned skin. Adding in PRP, a serum or growth factor to the procedure so that you end results is amplified. Microneedling is best when done in a package of 3-6 treatments every 4-6 weeks. The results with that treatment plan will be the best and you will love the results. iS CLINICAL Prodigy Chemical Peels The�Prodigy Peel System P2�combines resorcinol, lactic acid, salicylic acid, and citric acid to target multiple skin concerns ranging from mild to moderate aging, mild acne and pigment irregularities. These acids are combined with a powerful �booster� which features an advanced form of salicylic acid, enhancing the benefits when compared to other peels. The unique formulation allows for a decreased chance of pigment irregularities and can it can therefore be used on all skin types, including darker skin tones. The�Prodigy Peel System P3�combines resorcinol, lactic acid, and salicylic acid together with a powerful booster that features an advanced form of salicylic acid, which provides a medium-depth chemical peel, which results in a brighter and more youthful complexion. Full-Face Rejuvenation (FFR) In the aesthetics world, a full-face rejuvenation (FFR) treatment is all the buzz right now as many are coming in to get a refreshed look, wanting to turn back the clock and “just have that glow!” We get clients who are ready to jump in, when it comes to this treatment because they know that they will get the results they are looking for in 1-2 sessions at AIYANA. What exactly does a FFR mean and entail? Let’s dive into the details. FFR Explained AIYANA Rejuvenate is our FFR treatment that addresses the whole face, or palette, if you will. A customized treatment plan that takes into consideration your desires and wants when it comes to filler. The assessment includes an in-depth discussion of your anti-aging goals, you desires for outcome and longevity of results. We will address your most concerning areas as well as areas that are starting to become a concern. We will discuss your expectations with treatment and plans in the near future. Every person is different and no 2 treatment plans are the same. It truly is a customized treatment with filler and subsequent visits utilizing neurotoxins (Botox, Dysport) and Sculptra. When planning for the future, it is a good rule of thumb to think of 4 appointments per year. Neurotoxins/filler, Neurotoxins, Neurotxins/Sculptra, and Neurotoxins. Each of these appointment is spaced apart 3-4 months for best results. What To Expect This appointment for the AIYANA Rejuvenate will last about 60-90 minutes, including a consult. Multiple injection techniques will be used to achieve the best results. Note: If OK’d by provider–� Avoid all blood thinning medications including: ibuprofen, excedrin, Alleve, fish oil, vitamin E, etc for 2 weeks prior to injections. Avoid alcohol for 3 days prior to injections.� After injections, you will experience some swelling and possible bruising. Plan accordingly. If you notice bruising, we will provide you with our INhance Post-Injection serum to help. You will be advised to not lay down for 4 hours, do not get a massage or wear anything that may compress the area, no massaging of area, sleep on your back for 48-72 hours and no strenuous exercise for 48 hours. If you received the COVID19 vaccine recently, please notify your provider. Results With the AIYANA Rejuvenate, results are pretty much immediate. Most filler treatments are this way, which is what makes filler such a fun treatment. The results can last up to 2 years with possible touchups needed. We’ve always believe that knowledge in the field and unlimited training opportunities to always keep learn and expanding will make the best in this practice. How Do You Know if Someone is a Professional? When it comes to aesthetics and your face, it might be best to leave it to those who know best. And, by know best, we mean are trained professionals in the field. The world of aesthetics is a myriad of estheticians, assistants, nurses, and medical professionals, and everyone else in between. We’ve always believed that knowledge in the field and continuously learning/educating and training will produce the best outcomes in this practice. What are the Treatments that Should be Done by a Licensed Professional? When answering this question, we look at the treatments that have the most risk involved. That includes Botox/dermal fillers, laser treatments, microneedling and chemical peels. Advanced Aestheticians can perform certain laser treatments, microneedling and chemical peels. Nurses can perform Botox/dermal fillers if they have been properly trained, through a training course. Medical Providers, such as MD, NP, PA can perform more invasive laster treatments and Botox/dermal fillers, as well as any of the above mentioned or other treatments. All in all, it is important to make sure you feel comfortable with your treatment and your provider. Being at ease and trusting those who are doing such procedures is paramount in your overall satisfaction. Just Because They Are a Professional… Human error is just that. Any human, no mater how trained or schooled with make errors. It’s just part of the process. But, the ones who do will also know how to fix their mistakes. This makes doing your research even more important and finding someone who is trained makes all the difference. In those training, they are also trained on what to do when things go wrong. All in all, scheduling a consult and discussing all of the above is important. You need to feel confident in your aesthetic professional, knowing that they have your best interest in mind and they are trained to do what they do. RN to Aesthetic NP

RN to Aesthetic NP My first job out of nursing school was working in Mother Baby/Postpartum/ Antepartum unit in the hospital. I quickly transitioned to Labor & Delivery where I found so much joy and passion in my career. So, why now aesthetics as an NP? I realized that the passion and joy in Labor & Delivery was derived from 1 factor. Happiness of my patients. I loved being a part of a momentous occasion in their lives. Of course, there were the extremely sad times, but those were far and few in between and honestly a rare occurrence. The joy and happiness that radiated from my patients after giving birth and having the opportunity to witness this over and over agin is a moment in my life I will never take for granted. I loved my job as a L&D RN. I honestly knew at that time in my life that my calling was to be a midwife. So strongly that I didn’t even think of any other path. When it was time to choose a focus during grad school, the FNP program was so competitive that they were only going to accept 3 students from our class of 30. The chances of getting in were slim and in reality that was not the path I wanted to take anyway, but why not. So I threw my application in knowing full well I was not going to be one of those 3. The midwifery track was not competitive so once I applied I was accepted. The only caveat, it had a couple year wait list. During this time, my husband was also applying to dental school. And, though he applied to all schools within 3 hours of my grad school, he threw his application out to Tufts in Boston as he thought it would be fun to be there while his cousin was there in school. The Lords Plan Well, the Lord always has a plan, and I can assure you it usually isn’t one you planned on, its better. At least that is how the Lord works in my life. To my greatest surprise, I was accepted into the FNP track. I cried. It was mostly tears of sadness because I knew that the Lord desired this track for me due to my families situation, but deep down it was not what I wanted. I had a burning desire to be a midwife, to assist moms birthing babies into this world. Looking back I can see exactly why that was not the plan for me. As a mother of 2 at this point, and desiring more children, becoming a midwife is not just a career, it is a lifestyle. It is a career that you devote so much effort and energy to, outside of your normal working hours. And, at that time, I could not wait 2-3 years before starting my program because my husband was accepted into Tufts in Boston and that meant at the least we would be living in 2 different states for 1 year. What a whirlwind of emotions, but something I am forever grateful for. As I progressed through my program, I realized that not only was I meant to be in this FNP track, but it was exactly where I was supposed to be. We did live 1 year apart during my last year of my program. I had 2 children at home, 1 in elementary school and 1 with a nanny and I was pregnant with our 3rd. I also worked sporadic nights and weekends at the local hospital in labor and delivery. Honestly, my nanny was an angel sent to assist me during this time. I would have never made it without her. At that time, my husband was living in Boston starting his first year of dental school. He would fly home about every 6 weeks to visit, then back to Boston. We survived, and only by the grace of God will I even know how, but we did. Thinking of that time now, it’s a blur, as I’m not sure how we did it. But, I do know that it was exactly what was supposed to happen. The first year of dental school is extremely challenging. My husband spent most of his days, 5 am to 10pm commuting to and from school and studying. It would have been so difficult to try to coordinate all of that while the two of us were in our studies. Choosing an NP career When I graduated from NP school, I decided that I needed to step away from Women’s health and see if there were other options out there. I had 2 job offers, 1 in a OB/Gyn office and another at a correctional facility in Boston. To everyones surprise, I chose the correctional facility. And, to this day, I am so glad I did. My mentor, an FNP who had been practicing for over 5 years, took me under her wing and taught me everything she knew. She honestly gave me the confidence that I have today to be in this field and to feel like I can do hard things. She was the best thing that happened to me and I am forever grateful for her. Of course, at the correctional facility I became the GYN lead and over the Women’s Clinic, which, naturally, I loved. After my 1.5 year stint at the correctional facility, I went on to work in Urgent Care. I was actually a MA in urgent care for years while in college before going back to grad school. So, it was a natural transition, and to this day I still moonlight in urgent care. Why aesthetics? So, after all this, why aesthetics? When we moved to Oregon after my husband graduated, I realized I wanted to take that leap into the aesthetics field. It had to do with my days in Labor & Delivery, believe it or not. Misconceptions… around aesthetics and more. Since I have been in aesthetic medicine, I have noticed how many misconceptions there are out there in regards to this practice. Many people believe that when you get TOX and fillers, you look like an over-plumped “fish face,” as they say. The misconception is that is NOT the product, that is actually the injector or client preference. There are providers out there whose main interest is making money, and they really don’t are how they do it. There are many clients out there who find themselves loving the way they look after a good treatment, and then going back for more in a short amount of time. In turn, they are getting too much filler in one area, i.e. over-plumped or too much TOX- that causes clients to not be able to make adequate facial expression, i.e. frozen face. Body dysmorphia is real, and it is certainly alive and well in aesthetics. These clients keep going back to the office, and the provider keeps accepting them as a client and continues to provide their treatment. This isn’t to say its all the providers fault or all the clients, but as a provider we are given the ability to make a clinical judgment and we are actually licensed and advised to do so. I’ve found myself in a position where I have to have the hard discussion with the client. I then realized that evaluating the client, not just physically, but also their mental health is imperative. I ask myself– Is this individual doing other treatments. If so, are they going elsewhere? What else are they having done? How often are they getting a treatment? I also think to myself — this individual is my walking billboard. Do I want them to represent me and my work? If the answer is no, then the answer to the client is no. Using my license to make clinical decisions is imperative, even in aesthetics. AIYANA was founded on the principle that beauty comes from within, naturally. I am not here to change the way you look. I am here to enhance the natural contours of your face, to give you a little plump where the plumping used to be or to smooth out those pesky lines that are getting deeper than they originally were. I find myself circling back to my mission over and over again while in practice and I know my clients appreciate it. It is critical to have ground to stand on as a practitioner, a mission that you fully believe in that you can reiterate to your patients. As a nurse practitioner, I have the ability to step into this space as a medical director as well as a practicing nurse injector. What is so great is the autonomy it affords me to build a practice and provide compete care for my clients. The certification/training for nurse injecting is pretty ambiguous, and really there is nothing solidified in law that prohibits anyone from doing injections in a lot of states. However, when it comes to patient safety, which is my utmost priority, it is imperative that practitioners have adequate training. It has been know that estheticians have been injecting in the past, and some may still be today. Injecting WITHOUT any medical training is so dang scary. Insert Groupon deals, or Botox for a few dollars a unit. Be aware, it is out there! It is advisable, if you are looking to get injections, to do your research. Ask about training and education. Ask about experience. It is so important that you know who you are going to and that they are going to have the highest standards and safety rating. Aesthetics does not deserve the bad reputation it has been given in the past. No longer are clients looking for that overfilled, super tight look.
